Tooth crowding is one of the most common orthodontic conditions we treat in children. Crowded teeth can make brushing and flossing difficult, affect the appearance of your child’s smile, and—when severe—can prevent permanent teeth from erupting properly.
When crowding becomes advanced, it may lead to a serious condition called tooth impaction, where a permanent tooth becomes trapped in the jaw and cannot come in on its own.
When Does Tooth Crowding Begin?
Tooth crowding most often becomes noticeable between ages 8 and 9, when permanent teeth begin to erupt. Because permanent teeth are larger than baby teeth, your child’s smile may appear straight at a younger age and then become crowded as adult teeth come in.
This is why early orthodontic monitoring is so important. Changes in spacing and alignment often happen quickly during this stage of development.
How Is Tooth Crowding Treated in Children?
Treatment depends on the severity of the crowding:
Mild Tooth Crowding
If your child’s crowding is mild, our orthodontists typically recommend:
- Monitoring growth and development
- Delaying treatment until the ideal age for braces or clear aligners
Mild crowding is often easily corrected later with traditional orthodontic treatment.
Moderate to Severe Tooth Crowding
If crowding is more advanced, early intervention may be recommended. This can include:
- The use of a spacer to create additional room
- Growth-guided treatment to prevent future complications
- Interceptive orthodontics to guide erupting permanent teeth

The Risk of Tooth Impaction
The most significant risk associated with severe tooth crowding in children is tooth impaction. When a tooth becomes impacted, it cannot erupt naturally and often requires:
- Surgical exposure of the tooth
- Followed by orthodontic treatment to guide it into position
Early identification of severe crowding can often prevent the need for surgery by creating space before impaction occurs.
